Every time you get a complaint or a Non Conformance situation, you must treat it on your management system. In order to be effective on the Corrective Action, you must identify the root cause. If you fail on the root cause, you will be solving something that it's not the cause of your problem.
A simple way to do it? 5 Why's.
In the 5 Why's approach, you ask 5 times: why did this happen? The first time, you get an answer. Then you ask why the result of the answer happened. If you do this up to 5 times, it's likelly you get the root cause.
If you treat it conveniently, you probabilly won't do it again.
